Shanghai employee medical insurance can now pay for flu shots from your carry-over balance at designated clinics.
Shanghai employee medical insurance can now pay for a flu shot without a fresh cash bill, as long as the money comes from your carry-over balance and the clinic is inside the designated system. Since 15 September 2026, the same funds can also pay for eligible relatives through family sharing.
For years, the personal medical insurance balance could feel abstract if you rarely visited a public hospital. This policy gives it a practical autumn job: paying for prevention, sometimes for you and sometimes for a child or parent whose own resident insurance has no personal account.
One Shanghai medical insurance card, two different pots
The policy turns on four characters: 历年结余, the balance carried over from previous years. It does not turn on the total number you see if that figure also includes money credited this year. Before booking, look for the carry-over line in your medical insurance account rather than assuming every yuan is available.
| Money or situation | Can it pay for the vaccine? | What to do |
|---|---|---|
| 当年账户 · current-year funds | No, not for this payment | Check for a separate carry-over line rather than the total alone |
| 历年结余 · carry-over funds | Yes, if sufficient and the clinic and vaccine qualify | Use the balance for the listed vaccine and injection fees |
| 家庭共济 · family sharing | Yes, with eligible carry-over funds in the family network | Set up the relationship in 随申办 before the visit |
| Balance runs short | You can still be vaccinated | Pay the uncovered difference by an accepted payment method |
If you recently joined Shanghai employee insurance, you may have little or no carry-over yet. Check the actual balance instead of guessing from your start date. And if a dose is already free or fully paid by another scheme, the personal account does not pay for the same amount again.
Family sharing is the part worth forwarding

Shanghai's family mutual-aid arrangement, 家庭共济, allows an eligible employee-insured family member's carry-over funds to pay for relatives in the family network. This matters for children and others on resident medical insurance, which does not create the same personal account balance.
- Open 随申办 and search 医保个人账户家庭共济.
- Create or join the family network and add eligible relatives using the requested details.
- Select 共济支付 where that option is offered.
- Check the carry-over balance. Family sharing does not turn current-year money into eligible money.
Do this before you are standing at the payment desk with a child who has decided the whole outing was a mistake.
The designated clinic decides whether 医保 can pay
The rule applies at Shanghai medical-insurance designated medical institutions, 医保定点医疗机构, offering the listed vaccination service. Do not assume a private international hospital participates just because it accepts your commercial insurance. Do not assume every community health centre stocks every vaccine either.
Use the Shanghai Health Commission clinic information or the vaccination listings in 随申办, 健康云 and the 上海疾控 WeChat account. Then call before a long trip. Ask three things: are you 医保定点 for this vaccination, do you have the vaccine today, and can you settle from 历年结余 or 家庭共济?
您好,我想预约流感疫苗。请问门诊今天是医保定点接种吗?可以用职工医保个人账户的历年结余或家庭共济支付吗?今天有适合我的疫苗库存吗?
English: I would like to book a flu shot. Is this a designated vaccination clinic today? Can I pay from carry-over employee medical insurance or family sharing? Do you currently have a suitable vaccine in stock?

The Shanghai flu-shot booking path to save
Open 随申办 or 健康云, then look for 智慧接种 and 预约接种. Choose 流感疫苗 and a designated clinic where that option is available. Menu names and appointment availability can vary.
Bring the identity document used for registration, usually your passport for international residents, and activate the electronic medical insurance credential, 电子医保凭证, if you plan to use it. At the clinic, confirm the vaccine and payment route before the injection. Afterwards, expect the standard thirty minute observation period.
If you have an allergy history, are pregnant, are immunocompromised or have another medical concern, let the clinician assess suitability rather than relying on a general guide.

November still counts, and the rule covers more than flu
China CDC's 2026 to 2027 technical guide says protective antibodies usually develop two to four weeks after flu vaccination. It recommends vaccination before the local season where possible, while also making clear that vaccination during the season remains useful. If September and October disappeared under work and school forms, November is still a sensible time to act.
The same payment rule reaches beyond flu. District authorities report 116 listed vaccine products, including HPV, shingles, pneumococcal and rabies vaccines. That number describes products in the procurement catalogue, not 116 diseases, and stock still depends on the clinic and the day.

This is not a free-vaccine announcement. It is a new way to use money already sitting in your medical insurance account.

Good to know
- Good for: international employees paying Shanghai medical insurance, and families who may be able to use an eligible relative's carry-over balance.
- What changed: from 15 September 2026, carry-over employee medical insurance can pay listed non-programme vaccine and injection fees at designated institutions.
- What does not qualify: current-year account money, non-designated providers, and amounts already free or fully covered by another scheme.
- For families: set up 家庭共济 in 随申办 before the appointment and select 共济支付 where offered.
- Before you leave: call about designation, stock and payment, and take your registration identity document and electronic medical insurance credential.
- Questions: vaccination and clinics, 12320. Medical insurance and family sharing, 12393.
Can I pay first and claim the money back later?
The policy describes settlement from the carry-over balance at a designated institution, not a general reimbursement promise for every receipt. Confirm the payment route before paying if you want to use these funds.
Does family sharing mean my relative can use my whole account?
No. It means eligible expenses can be paid under the family mutual-aid rules. For vaccines, the same conditions still apply: a listed product, a designated institution and carry-over rather than current-year funds.
Can commercial insurance and 医保 both cover the same shot?
Do not assume two policies will reimburse the same expense. Ask the clinic and your commercial insurer how they coordinate payment, and keep the itemised receipt for any separate claim.
Where can I check which vaccine products are covered?
Shanghai uses the non-programme vaccine procurement catalogue published through the city's GPO service. The clinic confirms the product it actually has and whether it can settle that item through medical insurance on the day.

The Book of Changes says, 君子以思患而豫防之: the wise person thinks of difficulty before it arrives and prepares. That is the quiet idea behind both vaccination and a policy that lets an old balance pay for it.
